This information is for patients who are on hormone (endocrine) therapy after completing their breast cancer treatment.
We are inviting you to register for our new portal, Patients Know Best (PKB), to make it easier for you to contact us if you have worries about your cancer. For example, if you have a new symptom or side effect that may be related to your cancer. Read more about Patients Know Best (PKB) and The Clatterbridge Cancer Centre.
You can send a message to us any time via PKB when you need us. It means you will never have to worry about finding the right phone number, waiting until office hours or wondering who to contact if the person who usually call is not in work that day.
How to register
- You will get an email or SMS text message inviting you to register for your Patients Know Best (PKB) account.
- Follow the instructions and register.
- Then you can contact us via PKB.
How to use PKB to contact us
When you register and log into PKB, you will find:
- A welcome pack from the Breast MyFollowUpPlan team
- Health and wellbeing questionnaire – you can use this form to contact us if you are worried about any non-urgent changes, symptoms or side effects that may be related to your cancer. Note: Do not use this form if you have an emergency or need urgent help or advice. Contact your GP or NHS 111 instead – or call 999 if it is a life-threatening emergency.
If you want to contact the Breast MyFollowUpPlan Team just:
- Complete the health and wellbeing questionnaire. The questions help you provide the information that the team will need so they can help you.
- Your message will automatically get to the right person – they will read your message and make sure you get the right advice or help.
- They will arrange a phone call and/or appointment if they need more information or think they need to see you in person.
- We aim to view and respond to your messages within 14 working days.
This change will not affect your care or your clinical team. We just want to make it easier for you to contact us when you need to:
- You can send a message any time via PKB by completing a health and wellbeing questionnaire.
- No need to wait until office hours and remember your symptoms – you can log into PKB and tell us your symptoms as they are happening.
- Instead of having to find the right phone number, you can just log into PKB.
- No delays if the person you usually contact is off work – your message will automatically reach another member of the team so they can get back to you within 14 days.
What to expect from PKB
PKB is a secure online portal where you can store and access your health records.
You can use it to share information with healthcare teams or to track your progress over time.
It is best to read letters and other messages we send you the day they arrive. If you do not open a message on PKB within 48 hours, the system will automatically send a paper copy by post.
If you can't go digital
We know that not everyone has internet access or a suitable device - for example, a smartphone, laptop or tablet.
If this applies to you, you can contact the MyFollowUpPlan Cancer Support Worker on 0151 556 5823.
