Subject Access Requests

If you are requesting a copy of your medical records you must also enclose a cheque for £10.00, if you are requesting a copy of any scans and a copy of your medical records you must enclose a cheque for £20.00.

All information we hold about you is dealt with under the Data Protection Act 1998.

This includes information relating to health records, employment and training records – anything which is limited to you as a person whether as a patient, employee or partner to the hospital of any kind.

When making a request for any personal copies please specify the type of information you wish to be made available to you and enclose proof of identity – e.g. a formal document, a household bill (detailing your name and address), your driving license or your passport.

Under the Data Protection Act 1998 we have 40 working days to provide the information you ask for.
